The things you already stated are most important....cleanliness and new fixtures and new towels (white usually works best - make sure they are thick for a spa like feel). Fresh paint will yield the best bang for your buck.
Most people want to pull the shower curtain closed to hide the tub fixtures. This is a mistake. It makes the room feel even smaller. I always recommend the curtain be pulled open to the side.
In a small bathroom I would not have any rugs. (chops the space into smaller pieces)

What you said. Clean and fresh. I just put all new toilet seats in a new listing, cleaned the tile grout, and changed the tub and tile caulk around the edges of shower stall. Wide angle lens needed to make any room larger and not sliced, cramped. Vertical turn of the camera for a floor to ceiling helps capture what you do have and please always railroad spike any toilet seat down to make sure it is not left up for the razzle dazzle show and tell.
