How about: (using you standard addendum form)

In reference to REPC between named buyer and named seller dated some date, it is agreed that:

Paragraph (whatever), section (whatever if there is a section) box is marked (yes or no).

Will that work for your purpose?

I would ask my broker how to correct the purchase agreement if a box were not marked and should have been marked.  

The issue with the AR community answering questions like these are some are claiming it borders on giving legal advice.  What happens if you get "bad" or incorrect advice and you act on that advice?  Think about that. Think about the consequences and impact it could have on your clients.

Yes, mark answer and have all parties initial. That is what I do, it prevents unecessary paperwork.
