It varies by state. Hope you never have to personally find out! If you're working with foreclosures as an agent, be prepared for a long process.

It depends on your state as each state law is different. Take NH, the lender can send a notice of default, post the foreclosure auction for 3 consecutive weeks and foreclose on the 4th week. Done

Every state is different regarding the procedure, but I noticed you are in UT, so I can say that you are in a Deed of Trust state, That means that foreclosure is non-judicial. You start the process by providing "notice" and then follow the steps required by your state. You do not have to go to court in most cases. We have a similar process here in AZ and we actually have services that you can hire that will handle the entire process very inexpensively. You may have something similar there.
Try doing an internet search on the subject. You will find several sites that detail the process for Utah.

You'll need to review the laws pertaining to foreclosure in your state they won't be the same Virignia's foreclosure is much different from neighboring states.

I think it is a credit swap thing of sorts where no money changes hands because nobody has the note or standing to make a claim.
Google MERS litigation.
