I would hope that these brokers have a very good E &O insurance in place.  I have no idea how they do it.  With every closing, we would check the paperwork to make sure everything is in place.  Ultimately we as brokers were responsible. A

It is impossible for one broker to properly supervise 500 agents. But then again I have seen a broker with 10 agents that wasn't watching either. Perhaps if systems were in place and assistant managers to keep a close check, then maybe an adequate job could be done.

I have met brokers who have one agent and it is one too many for them.  I have met others with thousands of agents and they could easily be responsible for thousands more because they have good systems, good managers and good training.

I think the agents need to have access to their Managing Broker.  there's nothing worse than working for a big company, but feeling like your stuck in a rowboat out  in the ocean all by yourself with no compass and nobody to talk to.

My office has about 90+ agents, and our competitors are much larger, some well over 500+ agents. All the brands are doing it, but technically only one managing broker, but perhaps systems, and teams cover the balance.

Absolutely. If a broker has too manya gents and many are newer to the business they have to make assisting brokers because many will need personal attention, not just office meetings.

I, for one, have no idea what 'proper supervision' looks like.

What I do know is the only guiding principle of the overwhelming number of brokers is "Pay the fee and you are OK with me."

The true answer to your question lies in the area response by real estate professionals when they see the agent on the other side is with the 'old bone yard' brokerage with over  3,000 agents and one broker. We know WE have to oversee both sides. 

Creating a two tiered agent designation based on annual transactions and limiting the number of interns (those with low transaction count) a broker can shelter would be a step in the right direction.

Depends on each individual. Although, I believe there is a certain limit to how many people a single can work with at the same time. It is necessary to hire managers at some point. It becomes the responsibility of the broker to supervise and follow closely anyone who works under him/her.

I think I know who you are writing about in your second sentence.  That agent does know where the office is but has never visited it.  The broker can still see the quality of the work (e.g. agreements), responsiveness of the agent and talk to the appropriate administrative personnel and rest of the management team.  Some brokerages have a management structure and I know of local brokerages that have 1,000 or 3,000 agents along with a number of managers.

When a broker has as many agents as you mentioned, there is no way they can successfully monitor their actions. In my state an employing/managing broker is taking on a lot of liability by having so many agents.
