I don't ask. People in my estimation don't like surveys and they'll lie on it so as to not hurt your feelings. My advice is "don't waste your time".

My company sends out a comprehensive questionnaire asking to rate from 1-5 on things like communication, accuracy of information, attention to detail, professionalism, product knowledge, ease of working with, honesty, entusiasm, problem solving ability, etc. It asks if they would refer the agent to friends and relatives, why they chose the agent, what stands out most about the agent, etc. We get a great level of thoughtful responses from our clients.
